DBT diary card.
What is a diary card? It’s a form that the patient in DBT fills out each week (outside of therapy) to record their feelings, urges, destructive actions, and DBT skills used.
-OTC stands for over the counter (medication).
-The blank columns are for destructive behaviors. Every BP doesn’t practice the same behavior. One BP might have a problem with one behavior while another BP doesn’t engage in that behavior at all. One BP’s card may include binging and fasting while other’s may contain lying or overspending. If you have too many ‘others’ for the box, just write the most important ones (one’s you are working on currently to improve). If you don’t have many destructive behaviors, you can write something you want to improve. For example, I usually have “work” as a column. This helps keep track of disassociation, when I don’t have many hours worked I know that I’m having problems functioning.
-‘Assignments for the week’ are usually given by your therapist in private session. Home work is very personal to the BP’s goals. Homework also tends to involve skills, they need to be practiced every week.
The rest is pretty self explanatory.
I think it’s because I tried shaving my legs for the first time since I stopped cutting and I nicked myself. Oh, God. I can feel the anxiety go up up up.

Not gonna lie I love my dbt therapist. I go through phases of not finding her helpful, but she stopped me from killing myself today. Actually, she did more than that, she totally changed my mood and gave up two of her breaks to have phone calls with me to keep me going, and she’s been texting me since 10am this morning. She makes me feel as though she’s not going to give up, and that makes me feel safe. Thanks, Rita. I love you.
